Required Forms for 2022-23

All forms are fillable pdfs.  Once filled out and signed online, please email back to sheron@harmonyusd.org

You may also print the forms, fill them out and return to the school office.

If you need a paper packet, please let the office know.

All required forms are due by August 16, 2022.

Important information regarding the National School Lunch Program (NSLP) forms:

While all meals are free for all students, the district needs an NSLP form from every family, regardless of qualification.  These will be sent home on the first day of school. This information is vital for the district to receive reimbursement for our cafeteria program, grants, and other funding.  Please look for that form and send it back to the office as soon as you get it.  Please note that this form needs to be returned as a hard copy with a "wet" signature, so it is not available as a fillable pdf.
